Doctor's scripts for example have long been considered safe, even though there are countless stories of people stealing script pads and writing illegal scripts on order. Someone I know who has dementia recently 'changed their will' writing out all of their children in favour of a recently new partner. I'm sure this is not uncommon, but if the key form of evidence is the signature of the person and a 'witness', that's pretty risky.
